Tell us what is missing or failing, and where you are. The introduction is free, and the practice that contacts you is the one that examines you, plans the treatment and writes the fee.
The request goes to an implant dentist whose offices cover the town you gave us - Chicago itself, or one of the surrounding communities we match across. If something in the request is unclear, we may come back to you for the missing detail first, so that the practice is not ringing you to ask what happened to which tooth.
The practice then contacts you to arrange an examination or consultation. Expect the first visit to be assessment rather than treatment: the plan, the imaging and the itemised fee all come after that appointment, not before it.
None of that is a clinical assessment, and none of it commits you to anything. It is the detail that lets a practice tell, before the appointment, whether your case is one it handles.
A request is not an emergency route: if you are in pain or have a swelling or a broken tooth that needs seeing today, ring a dental practice directly rather than sending a form.
It is also not a way of obtaining a fee without an examination, a diagnosis, or a second opinion on clinical work - none of those can be given from a form, by us or by any practice. And it is not a complaints channel: if you have a concern about treatment you have already received, it goes to that practice, and licensing matters go to the Illinois Department of Financial and Professional Regulation.
The details you send are used to make the introduction, and they are passed to the practice being introduced to you so that it can contact you about your request. That is the whole of it: used for the match, shared with that practice, not sold on to anyone else.
If you want the request withdrawn or your details removed, say so and we will do that. If you would rather not be telephoned at all, put that in the message and we will arrange a different route.
This service publishes no phone number and there is no call centre behind it, so the form is the way in. It exists so that the practice receiving the request has your situation in writing before it picks up the phone - which is also why the detail above matters more than brevity.
If you are dealing with a dental emergency rather than a plan for missing teeth, contact a dental practice directly; a matching request is not an emergency route.
One or more, depending on your town and what the situation involves. If the first introduction is not right, tell us and we will suggest another at no charge.
No - a form cannot produce a fee. The quote comes from the practice after it has examined you and seen your imaging, in writing and itemised, so that it can be compared with another practice's plan.
